Monday-Friday Inpatient Case Management  1 weekend day per month

Occ Summary

Manage a designated caseload to coord inateandcomplete timelyassessment, planning, implementation and evaluati onofdischarge plansand care transitions across the continuum of care. E nsureoptimumutilization of the patient's and the Health System's resourc esandperform these duties within the requirements of CMS and other exter nalreview agencies.

 

Work Performed

 

Monitor daily cens usandassignment to assure all patients are assessedfor case managementn eedsincluding care coordination/transition anddischarge planning, consul tations, advocacy, education.Maintain effective communication with healt hcareteam members relatedto assessment findings, discharge planning need sand provider ordersneeded to arrange Homecare, Durable Medical Equipmen t,Transportation,Skilled Nursing or Acute Rehab Facility placement, Subs tance AbuseTreatment and outpatient follow-up.Maintain working knowledge ofspecific benefits and reimbursementguidelines, the discharge planningp rocess and applicable federal, stateand local regulations. Provideeduca tion and guidance on these topicsto providers, patients and families asn eeded.Work with Utilization Management partners to provide informationan dfeedback that will enhance negotiations and denial prevention withpayer s.Maintain timely documentation of assessment findings, dischargearrange ments andactions taken according to departmental guidelines;preparerepo rts and maintain records as requested and/or required.Participate inQual ity Assurance/Performance Improvement (QAPI)activities as requested.Jobr esponsibilities may include site specific duties, weekend/holidayor afte rhours coverage as designated at each entity.Develop and maintainpositiv e working relationships with customersinternal and externalto Duke Healt h System.Perform other related duties incidental to the work describedhe rein.

Requires Case Management Certification (ACM, CCM or ANCC) within 3 years or of hire. BSN required and must have current or compact RN licensure in state of NC. MSW(if no BSN or compact RN licensure in NC)
