Program Manager - Duke Primary Care 

 

As the Program Manager, you will lead operational excellence across Duke Primary Care (DPC), driving the success of DPC and its Population Health Program with a focus on behavioral health. You will provide strategic planning, project management, and administrative oversight to develop, implement, and optimize programs that align with organizational goals. This role partners closely with leaders and stakeholders to coordinate resources, monitor performance, and deliver measurable improvements in population health outcomes.

Key Responsibilities:

  • Directly manage the daily operations of a multidisciplinary team, including Behavioral Health Care Managers (RNs), LCSWs, and Licensed Counselors.
  • Handle personnel actions such as hiring, performance reviews, disciplinary matters, and other staff-related activities, while keeping accurate records and reports for program evaluation.
  • Monitor staffing levels and productivity, ensuring staff are meeting expectations and accountable for their work.
  • Develop, manage, and coordinate Population Health programs to ensure they achieve intended outcomes.
  • Work closely with the Director and Associate Director of Population Health and the DPC leadership team to deliver behavioral health services, support performance improvement, implement population health strategies, and contribute to DPC’s mission and Duke Health’s strategic goals.
  • Lead DPC’s Behavioral Health initiatives by identifying opportunities, planning improvements, and developing enhanced care delivery models to meet and exceed clinical, patient experience, and population health targets.
  • Encourage innovation and guide ideas from concept to implementation, overseeing project execution, communicating with leadership, identifying risks, and resolving issues as they arise.
  • Drive change across a distributed network of practice sites by building relationships, securing buy-in, anticipating obstacles, and creating strategies to meet project goals and timelines.
  • Remove barriers and influence stakeholders to ensure project success, establishing structures to support transformation efforts.
  • Support the development and use of data-driven insights and business intelligence for Population Health initiatives, creating dashboards and communicating progress and challenges to executive leadership.
  • Apply Lean, Six Sigma, and PDSA methods, and develop improvement skills training programs to foster a culture of continuous improvement.

Strategic Projects:

  • Access to Behavioral Health Resources: Design processes, build infrastructure, and create training to ensure patients, their families, and providers have equitable access to behavioral health resources.
  • Patient Experience: Review and improve workflows, and provide training focused on empathy and patient-centered care.
  • Behavioral Health Program Development: Design programs guided by patient needs to support value-based care models.
  • Care Team Cohesion: Develop strategies to strengthen interdisciplinary collaboration and patient-centered care.
  • Electronic Health Record (EHR) Enhancements: Work with the Population Health Data/Analytics team to design workflows and documentation that improve efficiency.
  • Impact: Use evidence-based population health models to create DPC-specific reporting that measures the impact of care on patient health and experience.
  • Behavioral Health Liaison: Collaborate across Duke Health with other behavioral health colleagues to advance the system-wide goal of delivering population health.

 

What you will need:

Education

  • Bachelor’s degree in a business or health-related field and Master’s prepared required.

 

Experience

  • Minimum of 3 years work experience, including 2 years of experience with significant responsibility for performance/process and quality improvement; as well as experience with population health analytics and application.
  • Clinical experience, working directly with patients in some capacity.
  • Experience leading work teams required.
  • Experience in oversight of programmatic quality, regulatory, and accreditation requirements is preferred.

 

Degrees, Licensure, and/or Certification

  • License in healthcare field is preferred. Certified Case Manager preferred. Lean Certification in Healthcare is preferred.

 

Knowledge, Skills, and Abilities

  • Effective written and verbal communication skill
  • Ability to communicate with stakeholders/staff/patients with diverse educational backgrounds
  • Analysis of data and processes for opportunities for improvement
  • Ability to manage numerous diverse projects simultaneously through effective priority setting, efficient use of time, organization
  • Knowledge of accreditation standards and regulations related to health care
  • Attention to detail and accuracy
  • Computer literacy and EHR competencies (specifically Epic/Maestro Care)
