General Description

The Population Health Specialist will work as a member of the QuEST team to assist with but not limited to auditing of patient electronic records, editing, revising or creating workflows for staff utilization, GEMBA walks of members of other teams, and assisting when needed with onboarding and training of new and current staff members hired with PHMO.

The Population Health Specialist is responsible and accountable for being organized with excellent time management skills, ability to be flexible and adapt to last minute changes or meetings being added to their schedule to support PHMO, good communication and written skills and ability to work with diverse group of individuals. The Population Health Specialist functions as an integral part of a diverse group of staff on the QuEST team that work on ensuring excellence in patient care, in an effort to achieve optimal clinical outcomes through a seamless model of access and care. Focus on quality management and education that are geared towards improving transitions in care for patients, physicians, family and community.

 

Duties and Responsibilities of this Level

Administer, interpret, provide both verbal and written recommendations on improving accuracy and sustainability of policies and identified workflows staying within accreditation standards.

Be able to contribute substantially and/or co-facilitate onboarding both in-person, virtually or through a learning management system, support QuEST team staff with other projects within their portfolio and work as an integral part of the QuEST team to identify areas of improvement in regards to patient care, transitions of care, workflow processes and education gaps.  

Participate in quality/performance improvement projects and provide presentations of outcomes in various group settings, including provider and practice meetings, as needed.

 

Required Qualifications at this Level


Education: Bachelor's degree in business, behavioral/social sciences, public health or related population health field.

Experience: 

Work requires three years of experience in a business, behavioral/ social sciences, public health or related population health field.

Sales and Marketing background, along with professional experience in Social Work, Disease Management, and experience working directly with Physicians and Advanced Practice Providers is strongly preferred.
