General Description of the Job Class

The clinical pharmacists will provide innovative pharmacy services to patients served by the Population Health Management Office (PHMO). Specifically, the clinical pharmacists will assist PHMO interdisciplinary teams in preventing initial inpatient admissions, preventing hospital   readmissions, and decreasing inpatient and outpatient spending. The clinical pharmacist shall serve as a resource for PHMO healthcare providers and staff.  A clinical practice may be maintained at a primary care clinic. The position also requires precepting pharmacy students and residents.

 

Duties and Responsibilities of this Level

Clinical Pharmacist Responsibilities

 

Clinical Practice:

 

  • Participates in clinic, care management activities, and provides clinical pharmacy services and oversight for safe use of medication. This includes prescribing and monitoring of medications within the scope of practice designed collaboratively with physician colleagues. This may include, but is not limited  to:

    • Completes/Reviews medication reconciliation

    • Provides medication counseling

    • Prescribe and adjust medications as permitted through collaborative practice agreement

    • Screens for drug interactions, therapeutic duplications, omissions, etc.

    • Monitors patients  for therapeutic  and/or adverse response to medication

    • Coordinates patient  care with other outpatient or inpatient  teams

    • Ensures continuity of care and access with all medications

    • Performs home visits for high-risk/high-priority  patients

    • Serves as resource to PHMO providers, care managers, and other healthcare professionals to provide medication reviews and to assist in medication reconciliation activities for high-risk PHMO patients.

    • Assists in identifying and developing interventions for PHMO patients that are not meeting PHMO target quality measures.

    • Serves as resource to PHMO network providers and care managers to assist in addressing medication access and medication adherence challenges.

    • Refers patients to patient assistance programs as needed.

    • Proactively assesses and ensures medication adherence.

    • Serves as a resource to PHMO providers and care managers for general drug information and Medicare pharmacy policy.

    • Provides educational in-services on medication-related topics to patients, PHMO care managers and other healthcare professionals.

    • May provide discharge counseling and education for select high-risk PHMO patients, either face-to-face or telephonically.

    • Provides both direct patient care and hospital follow-up telephonic services to PHMO patients as determined with consultation with PHMO Chief Medical Officer.

    • Provides support to improve safety and to reduce risk to patients. Reports events and/or concerns to SRS system.

    • Provides oversight of cost-effective use of medications and treatments. This includes risk reduction and correct utilization of resources to optimize care.

    • Provides leadership in developing guidelines, order forms, and critical pathways to improve the care and safety of patients.

    • Provides leadership to maintain TJC accreditation standards within the realm of medication use at clinical practice site.

    • Represents specific concerns for Information Technology applications.

    • Facilitates prescription fulfillment through hospital retail pharmacies, or other pharmacies as requested by the patient.

    • Develop and participate in pharmacy-related research projects and quality improvement initiatives for PHMO  patients.

    • Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of drug therapies

    • Coordinates effort to provide timely drug information to staff, upon request

    • Provides leadership support for research efforts in patient care areas.

    • Presents scholarly materials at educational conferences for pharmacy professional meetings  and organizations  (State/Local, National)

 

Scholarship Responsibilities

 

  • Maintenance of an active clinical research program, including mentorship of research projects for Residents and AHEC CSP students.

  • Publish 1 article/year in peer-reviewed literature appropriate for practice area.

  • Participate in local, regional and national professional societies, including presentations to health care professionals on practice-related pharmacotherapeutic topics.

 

Administrative Practice:

 

  • Participates in departmental, hospital, and PHMO committees and initiatives related  to primary care.

 

  • Works with peers to achieve division and departmental goals

 

  • Coordinates with payers to maintain up-to-date knowledge of benefit design and access to any payer-based data systems in support of pharmacy practice

 

  • Develops and implements grant-funded programs.

 

  • Contributes to quality reporting efforts required by payers.

 

  • Maintains familiarity with all departmental programs and works as integral part of the pharmacy team.

 

  • Models behaviors that exemplify hospital organization’s guiding principles, including trustworthiness, respect, diversity, learning and teamwork.

 

  • Maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy  services

Knowledge, Skills and Abilities

  • Ability to communicate clearly in oral and written form.

  • Extensive knowledge of drug products, medical therapeutics, pharmacokinetics, and disease states. Ability to work with information systems, automated and technical equipment.

  • Ability to establish and maintain working relationships with various members of health care teams

Minimum Qualifications

 

Education

Doctor of Pharmacy Degree OR BS Pharmacy Degree

 

Experience

  • With a BS Pharmacy degree, two years of pharmacy practice experience, or acceptable board certification, or completion of an accredited residency program and demonstrated ability to achieve positive patient outcomes in a similar environment is required.

  • With Doctor of Pharmacy degree, no experience is required.

 

Degrees, Licensures, Certifications

  • Licensed Pharmacist or eligible to practice pharmacy in North Carolina
