General Description of the Job Class

The Population Health Pharmacy Tech position is responsible for performing administrative tasks associated with the Medication Management for the Population Health Management Office (PHMO). They will be responsible for assisting the Care Managers and clinical pharmacists with medication-related issues such as medication adherence, medication access, medication histories, and other duties as assigned by the Pharmacy Technician Supervisor or Assistant Director of Population Health Pharmacy Services.

 

Duties and Responsibilities of this Level

Pharmacy Technician Responsibilities:

  • Work with PHMO clinical pharmacists on the PHMO’s medication management process and make revisions to the processes as needed.
  • Facilitate the completion of medication reconciliation activities, assist patients overcome barriers to medication adherence, conduct medication adherence outreach, complete community resource referrals and internal referrals to PHMO staff.
  • Obtain medication histories, including fill history, for medically complex patients from the patient, electronic medical record and any other available resources.
  • Communicate and collaborate with local pharmacies to explain PHMO programs and troubleshoot issues affecting medication adherence or access.
  • Conduct home visits independently or with a population health care manager (CM) to gather medication information and provide technical assistance and support to CMs and clinical pharmacists.
  • Track metrics, as needed under the direction of the Lead Population Health Pharmacy Technician, relating to the population health pharmacy program.  This may include, but would not be limited to, following PHMO pharmacy processes for documenting technician interventions in the electronic medical record.
  • Maintain a basic understanding of the NC Medicaid PDL, applicable payer formularies and pharmacy benefits, and the prior authorization process for Medicaid and other common payers served by the DukeWELL pharmacy team.
  • Document all patient interactions and activities in Maestro Care or other system as requested by the Lead Population Health Pharmacy Technician, the Director of Population Health Pharmacy Services, and/or PHMO leadership. 
  • Assist with special population health pharmacy initiatives and projects as needed.
  • Utilize available databases and software to assess patient activity/medication adherence and address barriers to care, when possible.
  • Participate in staff meetings, in-services, and other program-specific tasks and activities as required.
  • Takes responsibility for personal and professional growth.

 

Knowledge, Skills, and Abilities:

  • Considerable knowledge of drug names, medical terminology and pharmaceutical calculations.
  • Proficient computer skills.
  • Strong written and verbal communication skills. 
  • Requires critical thinking skills with ability to implement solutions.
  • Excellent customer service skills including prioritizing the needs of patients and multidisciplinary team members while displaying empathy in all interactions to ensure complete satisfaction.
  • Strong organizational skills including staying focused on assigned task and using time and resources effectively and efficiently to achieve job expectations.
  • Typical physical demands include ability to perform the job while standing, reaching, stretching, stooping, and sitting for prolonged times; manual dexterity sufficient to perform computer, dispensing functions of the job; ability to lift up to 20 lbs., while walking, stooping, bending, and reaching.

 

Minimum Qualifications

 

Education

  • High school diploma or equivalent and completion of an accredited pharmacy technician-training program. or Acceptance into pharmacy school. or High school diploma or equivalent and one year of pharmacy or health care related experience.

 

Experience

  • Minimum of one year related experience required

 

Degrees, Licensures, Certifications

  • North Carolina Board of Pharmacy registration is required.

  • Pharmacy Technician certification is required from the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
