General Description:
The Continuity of Care pharmacy technician is a self-motivated, independent pharmacy technician responsible for working collaboratively with the Transitions of Care Manager within the Clinical and Patient Care Services division to perform medication history interviews for patients at Duke University Hospital.
Work Performed
- Use high-level communication skills to perform a patient interview and obtain a complete home medication list. This includes contacting retail pharmacies or physician offices to verify the medication history with persistence to achieve the highest level of accuracy.
- Ensure complete and accurate documentation for each identified medication in the electronic medical record with careful attention to detail.
- Discuss any identified issues related to the medication history with the clinical pharmacist staff to promote high-quality transitions of care.
- Provide medication history related pharmacy technician and student training and development.
- Participate in departmental goal planning and quality improvement activities, focused on care transitions
- Participate in committee, workgroup or team meetings Contribute to development projects (design, implementation, measuring impact)
- Maintain familiarity with all departmental programs Perform other related duties incidental to work described herein
Knowledge, Skills and Abilities
- Knowledge of medication names (brand/generic) and basic math
- Attention to detail and persistence in work ethic
- 5 years (Community/Retail setting preferred)
- Experience performing medication histories/reconciliation is preferred
- Able to work independently and prioritize responsibilities in a high-volume environment
- Necessary communication and interpersonal skills to maintain effective working relationships with patients and co-workers
- Takes responsibility for personal and professional growth
- Must be able to operate a computer, telephone, pager and other pharmacy
Minimum Qualifications
Education
- High school diploma or equivalent and completion of an accredited pharmacy technician-training program. or Acceptance into pharmacy school. or High school diploma or equivalent and one year of pharmacy or health care related experience.
Experience
- Minimum of one year related experience.
Degrees, Licensures, Certifications
- North Carolina Board of Pharmacy registration is required.
- Pharmacy Technician certification is required from the Pharmacy Technician Certification Board (PTCB) or The Exam for the Certification of Pharmacy Technicians (ExCPT).
