Job Summary

Hematology Consult Service activity (approximately 20-25% effort): Hematology is searching for an APP to assist with the hospitalized hematology consult patient service. The APP will round daily on the hematology consult service team and would be responsible for seeing several new patients each day as well as having a group of follow-up patients seen each day. Timely and appropriate management of treatment related side effects would also be facilitated by this individual.

 

Hematology Engagement Center Clinical Activity (approximately 75-80% effort):

Nurses and CMAs in the Engagement Center provide telephone triage, in-basket management, medication refills, and prior authorizations. The APP in this role would be the primary contact for the Engagement Center to escalate clinical interactions with patients for clinical evaluation and management. The APP could be based at the Engagement Center or work remotely.

 

Primary hours will be Monday through Friday, 8am-5pm, with hours flexible to meet the needs of the patients. No required weekend or holiday coverage.

 

Job Duties & Responsibilities Related to the Hematology Consult Service activity:

  • Daily rounding with the Hematology Consult Service to include presentation of new and follow-up patients as indicated; round with the Hematology attending on patients who need to be seen by the attending; coordinate efforts with the Hematology fellow on service to divide up new Hematology consults who will be followed by the APP; coordinate efforts to provide coverage (with the Attending) for the Hematology fellow when they are in their outpatient clinic.
  • Take patient histories, conduct physical examinations, and record the data in the patient’s record; write progress notes; prepare patient workups and summaries noting pertinent positive and negative findings; obtain psychosocial history relevant to patient care, and assume responsibility for developing therapeutic relationships with patients.
  • Order and schedule relevant laboratory studies and diagnostic procedures for the workup of hematologic disorders seen in the inpatient setting; explain necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
  • Perform bone marrow aspirates and biopsies on patients who need this procedure as part of their diagnostic evaluation.
  • Coordinate efforts with the Hematology outpatient triage service to facilitate discharge from the hospital with timely outpatient follow-up with the appropriate provider.
  • Share pager coverage with the fellow to provide coverage for medication approvals, follow-up on laboratory results, etc.

 

Job Duties & Responsibilities Related to the Engagement Center Clinical activity:

  • Regular huddles with the Engagement Center team to query about requests and tasks being managed by the team.
  • Review and manage escalated questions from the Engagement Center, triaging patients who can be directly managed and who need to be escalated to the patient’s primary provider.
  • For patients calling in with questions, the APP can convert these inquiries into e-Provider telehealth visits for detailed review and assessment. The APP may bill for these services.
  • When necessary, arrange for outpatient laboratory tests and/or imaging studies to assess individual patients as needed.
  • Coordinating with outpatient providers when patients need to be seen in-person.
  • Providing brief follow-up to the outpatient provider for services rendered entirely through the Engagement Center.

Job Eligibility Requirements

 

Education           

  • Nurse Practitioners:  Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
  • Physician Assistants:  Work requires completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.

 

Certification 

  • BLS required.

 

License                

  • Nurse Practitioners:  Must have current or compact RN licensure & NP approval to practice in the state of North Carolina. BLS required. National certification as a NP required.
  • Physician Assistants:  Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants. BLS required.

 

Experience          

  • 1+ years of APP experience strongly preferred

 

 

Job Hours

Monday-Friday, 8am-5pm, with flexibility to meet clinic needs.  No required weekend or holiday coverage.
