Duke GI Advanced Liver Disease Clinics, 40 Duke Medicine Circle, Durham, NC

 

Job Summary

We are seeking an Advanced Practice Provider in our Duke GI Clinic located in Durham, NC to work in collaboration with a team of 9 attending physicians and 3 Advanced Practice Providers to collect and document data, conduct diagnostic and therapeutic procedures, order and schedule laboratory studies and professional consultations, and provide direct patient care services to adult patients with advanced liver disease. Primary hours will be Monday through Friday, day shift, with hours flexible to meet the needs of the patients.

 

Duties may include but are not limited to those listed below and may include other outpatient procedures or services as deemed appropriate by the supervising physician. Current position would be to hold 7 half day clinical sessions per week. These sessions can include independent advanced liver disease clinic patients, joint sessions with the advanced liver disease physician providers and may include procedural sessions including paracentesis and fibroscans in an outpatient hospital-based clinic that provides urgent care to patients with liver disease.

 

Job Duties & Responsibilities

  • Record the history of present illness, past medical, family and social history
  • Perform physical examinations
  • Perform fibroscan assessments (APP will be trained to perform this ultrasound-based technology)
  • Perform paracentesis (APP will be trained to perform this procedure)
  • Establish and follow plan of care in conjunction with MD
  • Prescribe appropriate medications to a specialty pharmacy that will handle prior authorization and other necessary paperwork; order refills as needed
  • Patient and family teaching
  • Order appropriate labs, tests and procedures and follow up with patient as needed
  • Coordinate care and refer to other medical professionals as needed
  • Document clinic, telephone, and other patient encounters in the Epic (Maestro) system
  • Review lab results in Maestro, communicate and interpret results
  • Patient triage and return patients calls within 24-48 hours; provide feedback regarding scheduling of urgent patient appointments
  • Review outside records including clinic notes, hospital records, outside radiographs, lab, and other test results
  • Collaborate with nursing staff to complete patient paperwork including medication pre-certifications for insurance companies, durable medical equipment requests, disability paperwork, and handicapped permit applications
  • Handling patient and referring physician phone calls in a timely manner, coordinating imaging, invasive procedures such as TIPS, interventional radiology management of hepatoma and liver biopsy, management of prior authorization of specialty meds; responding to requests for prescription refills and pharmacy requests
  • Responsible for patient follow up regarding pathology and laboratory studies, follow up and entry of QI data for service;
  • Attendance at GI Grand Rounds, weekly Liver Transplant Selection conference, quarterly Liver Transplant Retreat, monthly transplant morbidity and mortality and quarterly liver quality and performance improvement conference is expected. Participation in weekly Multidisciplinary Radiology Conference and Hepatology Pathology Conference is expected when cases managed by the APP are to be presented.  Participation in other divisional educational events is also encouraged.

 

Job Eligibility Requirements

 

Education           

  • Nurse Practitioners:  Work requires completion of an accredited Nurse Practitioner program, with a Master's degree in Nursing.
  • Physician Assistants:  Completion of an accredited Physician Assistant program. A Bachelor's degree required; Master's degree strongly preferred.

 

Certification 

  • BLS required.

 

License                

  • Nurse Practitioners:  Must have current or compact RN licensure in the state of North Carolina. BLS required. National certification as a NP required prior to or within 6 months of employment.
  • Physician Assistants:  Must be licensed in the state of NC, and be certified as a Physician Assistant by the National Commission on Certification of Physician Assistants.

 

Experience          

  • 1+ years of APP experience strongly preferred.
