Combination of home-based patient care and Hospice Inpatient Unit (Hock Pavilion)
JOB SUMMARY:
Collaboratively coordinates and provides care of the hospice patient population. Assesses staff educational needs related to the provision of clinical care services. Defines appropriate staff competencies in collaboration with the Duke HomeCare and Hospice (DHCH) Medical Director. Provides care and performs medical acts as a Nurse Practitioner, under the supervision of the supervisory physician, in the home or hospice inpatient care facility or partner facilities.
JOB DUTIES AND RESPONSIBILITIES:
- Provide direct patient care and may perform medical acts as a nurse practitioner under the supervision of the supervising physician in the home, community-based care setting, or hospice inpatient care facility or partner facilities. The supervising physician will be available by pager, mobile phone, or other electronic means.
- Assist in the coordination and development of the care plan collaboratively with the clinical staff, patient, family, physician and medical director, and establishes mutual goals.
- Assess and provide patient/family teaching based on patient and caregiver’s needs.
- Participate in on-call rotation for medical coverage of hospice.
- Participate in weekly interdisciplinary patient care conferences. Establishes, reviews, updates, and summarizes care plans in keeping with Medicare CoPs and Licensure requirements.
- Partners with DHCH to identify learning needs of DHCH Staff and provides appropriate education in collaboration with the Directors of Patient Services/Hospice/Home Health/Infusion and the Clinical Nurse Educators.
- Participate in community educational projects.
- Participate in the performance Improvement Program of DHCH.
- Perform complex diagnostic and therapeutic procedures in diagnosing medical and surgical problems and prescribing necessary treatment and services for quality patient care; record normal and abnormal data and tentative conclusions for review by the physician.
- Coordinate and schedule laboratory studies and diagnostic procedures; explain the necessity, preparation, nature and anticipated effects of scheduled diagnostic and therapeutic procedures to the patient.
- Monitor patient’s status, maintain a record of the patient’s condition and consult with supervising physician when patient’s response does not meet anticipated and/or predetermined criteria.
- Participate in basic science and clinical research projects conducted by supervising physician; prepare statistical reports and analyses setting forth progress, adverse trends and appropriate recommendations or conclusions.
- Assist in teaching students, residents and fellows in complex techniques and procedures which are unique to specific clinical setting.
- Utilize current nursing and clinical research to support evidence based clinical decision making and practice. Participate in strategic growth planning to expand hospice and palliative care services in the communities served by DHCH.
- Demonstrate responsibility for professional practice through active participation in professional organizations, continuing education and when applicable in obtaining certification.
- Maintain confidentiality of employee and patient information.

- Participate in Quality Assurance Performance Improvement (QAPI).
- Promote quality, comprehensive services through a team approach.

JOB ELIGIBILITY REQUIREMENTS
Education
- Graduate of accredited MSN or DNP Nurse Practitioner program for adult care.
Certification
- Any NP certification that treats adults (FNP, ANP, AGNP, AGACNP, etc.)
- Federal and State DEA independent license/prescriptive authority
License
- Must have current RN licensure issued by or recognized by the NCBON with authorization to practice as NP
Experience
- Two years’ experience as a Nurse Practitioner strongly preferred.
- Previous Home health or hospice experience preferred.
Physical and mental requirements:
- Able to clearly articulate scope of practice, and practices within those guidelines as a Nurse Practitioner / Physician Assistant in the state of North Carolina and within the Duke University Health System.
- Able to independently seek out resources and work collaboratively to solve complex problems
- Able to communicate clearly with patients, families, visitors, healthcare team, physicians, administrators, leadership, and others.
- Able to use sensory and cognitive functions to process and prioritize information, perform health assessments, treatment, and follow-up.
- Able to use fine motor skills
- Able to record activities, document assessments & interventions; prepare reports and presentations;
- Able to use computer and learn new software programs
- Able to provide leadership in clinical area of expertise and in meeting organizational goals
- Able to navigate the Hospital and DUHS to provide clinical expertise to specific patient populations
- Able to withstand prolonged standing and walking with the ability to move or lift at least fifty pounds
- Able to remain focused and organized
- Able to work collaboratively with all levels of personnel
Job Hours
- Weekdays: 10-hour shifts four days a week: 8am – 6pm
- Overnight call one night per weekday
