Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

JOB SUMMARY: patient.as assigned.Provide and document direct nursingcare of assigned patients under thesupervision of aregistered nu rseorphysician. Nursing care is guided by medical andnursing plan of car eestablished policies and procedures. Patient response to care to berepo rtedto a registered nursefor evaluation, intervention and modification o ftheplan of care.MAJOR JOB FUNCTIONS:1. Provides nursing care for patien tsin accordance withphysician#s order and established policies and proce dures.2. Participates in weekly team meetings and case conferences,asnec essary, to facilitate thatappropriate services are provided to the3. Mai ntains up to date knowledge of Medicare/Medicaid/JCHO thru4. Inservices and workshop attendance.5. Coordinates care plan activities with the mul tidisciplinary teammembers as directed by the RNcase manager.6. Assess t heseverity of patient#s symptoms and communicates findingsto supervising RNandcommunicates with MD as appropriate and directed by the RN.7. Docum ents care and services provided in accordance with DHCHpolicies and proc edures andregulatory standards.8. Required documentation is timely, conc ise and complete.9. Follows standard precautions and uses personal prote ctiveequipment as required.10. Observes patient#s reactions to medicatio ns and reports any adversereactionsto the supervising nurse and physicia n.11. Communicates regularly withteam members, caregivers andphysicians. 12. Identifies personal learning needs and implements correctiveaction.1 3. Implements agency policy, procedures and objectives.14. Maintains con fidentiality ofemployee and patient information.15. Exhibits an attitude which promotes harmony and goodwill in theworkplace.16. Displays a clean ,neat, professional appearance.17. Participates in Performance Improveme nt Program.18. Promotes quality, comprehensive services through a teamap proach.19. Performs other duties
