Duke HomeCare, Hospice, and Infusion Services (DHCH) Referral Center 

Shifts: PRN Position with expected every other weekend and potential weekday PTO coverage.

The LPN in the Duke HomeCare, Hospice, and Infusion Services Referral Center serves as a liaison between hospitals, physician offices, and our service lines, ensuring the seamless coordination of home health, hospice, and infusion services referrals. This office-based role requires strong clerical and administrative skills, including electronic medical record (EMR) management, ICD-10 coding, and order entry.

  • The LPN is responsible for finalizing referrals by reviewing admission documentation, verifying the appropriateness of services, and collaborating with referral sources. Additional responsibilities include responding to customer inquiries, assisting with scheduling, and maintaining accurate patient records.
  • Success in this role requires problem-solving, analytical thinking, and critical decision-making to navigate complex referrals and ensure compliance with regulatory guidelines. Given the dynamic nature of the business, flexibility and adaptability are essential, as priorities may shift based on patient needs, referral volume, and evolving organizational goals.

Duties and Responsibilities of this Level:

Referral Coordination:

  • Transcribe incoming referral orders.
  • Review orders and patient charts for key information needed to process referrals.
  • Determine if the patient meets the criteria for admission.
  • Create and maintain patient charts.
  • Utilize ICD-10 coding to enter accurate start-of-care diagnoses
  • Work within the RightFax folder and appropriately attach documents in EMR.

Communication and Inquiries:

  • Handle incoming phone calls into the referral center.
  • Assist callers with inquiries about the types of services our agency provides, how to access our services, the status of their referral, appointment scheduling, insurance acceptance, and referral confirmations from doctors.
  • Provide excellent customer service to internal and external customers.
  • Serve as an educational resource to referral sources and patients/families regarding home care coordination and available services.

Collaboration:

  • Work closely with physicians, caregivers, and other healthcare personnel to ensure accurate information exchange.
  • Collaborate with other department leads to clarify specific patient criteria.
  • Communicate regularly with team members to ensure smooth referral processing.

Administrative Support:

  • Participate in the development and implementation of general policies and procedures.
  • Assist in the training of new employees.
  • Maintain up-to-date knowledge of federal and state regulatory guidelines related to home care services.
  • Ensure accurate documentation and EMR entry, including order entry and patient information management.

Work Environment & Scheduling:

  • Some weekend scheduled on-call backup may be required.
  • Ability to work efficiently in a fast-paced environment and handle competing priorities.
  • Willingness to work overtime when necessary to ensure referrals are completed before the end of the shift.

Required Qualifications at this Level

Education:

  • Graduation from an accredited Practical Nurse program.

Experience:

  • Preferred experience in a Home Health setting for 1+ years.
  • Experience working with EMR systems.
  • Knowledge of disease processes and medical terminology is advantageous.

Degrees, Licensure, and/or Certification:

  • Current State of North Carolina LPN licensure is required.

Knowledge, Skills, and Abilities:

  • Proficiency in electronic medical records (EMR) documentation, order entry, and data management.
  • Strong knowledge of ICD-10 coding to ensure accurate patient diagnoses.
  • Understanding of home health, hospice, and infusion services regulations, including JCAHO compliance.
  • Excellent attention to detail, organizational skills, and ability to handle multiple priorities.
  • Strong computer skills and ability to learn new software programs.
  • Effective communication skills with patients, families, referral sources, and healthcare team members.
  • Ability to independently seek out resources and work collaboratively.
  • Strong customer service skills and problem-solving abilities.
  • Adaptability to changes in workflow to accommodate patient care.
  • Capacity to remain focused and organized in a high-volume, fast-paced setting.

This position leverages the clinical knowledge and background of an LPN to perform essential clerical and administrative tasks, ensuring seamless coordination and delivery of patient care services.
