As the Community Health Worker, you’ll be a vital part of a collaborative, interdisciplinary team within the Sanford School of Public Policy’s Center for Child and Family Policy. You’ll support residents receiving services through Durham Housing Authority’s Durham Choice Initiative, helping to unlock opportunities for children and families in our community.

What You’ll Do:

  • Resident Engagement and Assessment (40%)
    Build trust and provide direct support to Durham Choice residents by assessing and addressing their social and health care needs.
  • Documentation and Data Entry (15%)
    Maintain accurate and confidential records of resident interactions and outcomes using data platforms to support program evaluation.
  • Interagency Coordination and Collaboration (20%)
    Work closely with outreach workers, family support specialists, and case managers to ensure comprehensive service delivery.
  • Resource Navigation and Referrals (15%)
    Help residents access community resources, complete benefit applications, and connect with external health and social services.
  • Supervision and Program Review (10%)
    Meet regularly with your supervisor to review progress, discuss challenges, and ensure quality documentation.

What You Bring:

  • Strong verbal and written communication skills
  • Organizational skills to maintain client records, track outcomes, manage schedules, and meet clients punctually
  • Computer literacy, including proficiency in typing and standard Microsoft applications
  • Ability to establish trust with residents, respect their needs, and maintain confidentiality
  • Education equivalent to a high school diploma
  • Preferred training: Community Health Worker or Nurse Assistant
  • One year of relevant experience assisting clients or patients
  • Familiarity with the Durham community is a plus
  • A valid North Carolina driver’s license with a good driving record
  • A commitment to equity, inclusion, and community engagement

 

Anticipated Pay Range:

Duke University provides an annual base salary range for this position as USD $18.00 to USD $24.57. Duke University considers factors such as (but not limited to) scope and responsibilities of the position; candidate's work experience, education/training, and key skills; internal peer equity; as well as market and organizational considerations when extending an offer.
