Clinical Service Nurse II

Job Summary:

We are seeking a CLINICAL SERVICE NURSE II to be an integral member of the healthcare team and provide direct and indirect care to patients. This position coordinates referrals and clinical program services, manages a caseload to provide care to our Med Peds complex care patient population, and assists providers with care of patients identified at Duke Children’s and Internal Medicine North Durham. This position will support more adult than pediatric patients.

 

Job Duties and Responsibilities:

  • Coordinates Clinical Practice and Clinical Program Services; manages a direct-care caseload to provide care to patient populations identified by the primary care physician.
  • Enter pre-visit orders and prepare forms (such as FMLA, FL2, PCS, CAP-C, CC4C, MAW and allergy/asthma action plans, etc.) as needed.
  • Assist with the development and maintenance of clinic Chronic Care Disease Registry.
  • Designs and Coordinates Education Programs for families and patients to assist with care outside of clinic.
  • Practice Development; Participates in various clinic committees to organize and improve patient care and serves as liaison between patients and providers to communicate needs and provide patient care.
  • Monitor Maestro in-basket for patient call notifications. Respond immediately to semi-urgent and urgent message received by pager, return patient calls, collaborate with triage & providers and communicate plan of care to patient. Document patient communication in telephone encounters.
  • Using the Nursing Process, collect and analyze patient data, make recommendations for nursing care plans, evaluate nursing care provided, document and communicate appropriately nursing actions taken and patient responses.
  • Serves as a resource to patients and families, providing counseling and follow-up problem solving.
  • Assists providers with patient appointments and provide onsite coordination in clinic.
  • Collaborates with social work, Duke Well, and other members of the patients care team for patient needs.
  • Coordinates the development, utilization, and evaluation of appropriate educational resources and interventions that assist the patient/caregiver in developing the skills for self-care.
  • Participates in quality/performance improvement programs and projects, product evaluation, outcomes evaluation studies and/or clinical research. 
  • Perform other related duties incidental to the work described herein.

 

Job Eligibility Requirements:

  • Work requires graduation from an accredited BSN program.
  • At least three years demonstrated clinical expertise in Newborn Nursing Care, Pediatric Primary Care, Parent Education and Telephone advice.
  • Must have a current or compact RN licensure in the state of North Carolina
  • BLS certification required.

 

Job Hours:

Our clinic’s hours of operation is Monday-Friday 8:00 am-5:00 pm.
