Occ Summary
Provide pharmaceutical care services to the patients of the DukeUniversity Health System including, but not limited tooversight,monitoring, preparation, dispensing and proper documentatio nofpatientmedication therapy as wellas providing drug information, pati entinformation and oversight of information and dispensing systems; ensu recompliance with Federal, State and local laws and regulations as well asproper storage and documentation. May also advise physicians on matter spertaining to drug usage and control; participate in the development an ddelivery of continuing education programs for Pharmacy, Patient CareSer vices, Medical Staff and students.
Work Performed
Rev iewand monitor patient's medication therapy and orders written byappropr iate caregivers for safety and efficacy considering specificpatient cond itions and health status; advise patient care team of issueswith, or pos sible enhancements to, patient's medication therapy anddocument such interventions. Prepare, label and dispense medications forpatients in accor dance with physicians' prescriptions; ensure compliancewith Federal, Sta teand local laws and regulations pertaining to thedispensing of drugs an dnarcotics and the maintenance of requiredrecords.Maintain appropriate r ecords of dispensing and oversee and maintain thesystems that charge orc redit for medications and medication treatments.Provide drug information and educate caregivers, patients and studentsregarding medications andth eir proper use and monitoring. Oversee andmonitor all aspects of themedi cation process; collect data and presentreports and recommendationsregar ding issues with the medicationsystems.Ensure that appropriate medicatio ns are available for patients andproperly procured, prepared, stored and maintained.Supervise the process and technical personnel who assist inpr eparation,maintenance and dispensing of drugs and supplies.Provide suppo rt, education and oversight for investigational medicationsfor patients; provide information regarding their preparation, labelingand availabids and documentation for investigational drugs.Provide specializedpharmali ty within the medication system; maintain appropriate andrequiredrecorce utical services to improve drug usage andtherapeutic outcomes including advising physicians on issues concerningdrug therapy, the inherent toxic ity of drugs and side effects, as wellas assisting in the prescriptionof appropriate doses.Contributes to the education of students and pharmacy residents.Maintain liaison relationships with medical and nursing staff ; providetimely information pertaining to pharmaceutical supplies, drug usage andcompatibility, state and federal regulations regarding drugcont rols,and Joint Commission standards.Conduct and evaluate medicationhisto ries, assess compliance and suggestmodifications so as to achieve desire d outcomes; instruct patients inthe proper use of prescribeddrugs;partic ipate in patient care roundswith multidisciplinary teams toevaluate pati ent progress.Individualize medication regimens using sound principles, a ccounting forpharmacodynamic and pharmacokinetic variations in drug abso rption,distribution, metabolism and elimination with responsibility fore stablishing and continually improvingthe delivery of pharmaceuticalcare to patients within areas of direct responsibility and assistingothers i n the department with the same.Conduct and participate inresearchincludi ng, but not limited to,performing as the primary investigator orco-inves tigator for researchprograms which will impact on thedelivery of quality care or examinethe pharmacoeconomic impact of providing care to the pat ients in theDuke Health System.Perform other relatedduties incidental to the work described herein.
