Clinical Nurse II- Duke Signature Care

Job Summary:
We are seeking a Clinical Nurse II in our Duke Signature Care clinic located in Durham, NC to be an integral member of the healthcare team and provide direct and indirect care to patients. Independently plan and provide professional nursing care for patients in ambulatory clinic settings in accordance with the medical and nursing plans of care and established policies and procedures. Provide nursing services to patients and families in accordance with the scope of the RN as defined by the North Carolina Board of Nursing.
Address patient phone calls and coordinate care needs in an ambulatory concierge practice/ Primary Care Medical Home. Includes, but not limited to, triage of acute care needs utilizing approved protocols when appropriate; obtaining medical documents; prescription refills; managing home health supplies and equipment; and scheduling patient visits.
Job Duties and Responsibilities:
- Uses proficient assessment skills to identify real and/or potential issues impacting patient status; communicates with physicians; schedules patient visits in clinic as needed.
- Effectively establishes, coordinates, and communicates priorities for patient care utilizing time management skills.
- Identifies, manages, and role models multi-tasking of multiple complex patient & family care needs safely.
- Recognizes and coordinates patient care needs with interdisciplinary team daily assisting in clinical flow with nurse specific tasks within clinic as needed.
- Use triage protocols as clinical guidelines in conjunction with critical thinking and judgement when documenting in the electronic record. Follow clinic specific protocols and standard work to support safety and best practice.
- Attend and engage in daily huddles/ performance improvement.
- Answer and address incoming phone calls, fax, and electronic communications to triage during clinic hours including addressing and clearing messages from the answering service work ques daily; manage acute patient needs in a timely fashion.
- Answer and address nurse triage inbasket pool messages including shared staff messages, medication refill requests, prior authorizations, and other patient specific messages from providers requiring patient follow up. Additionally, provide ongoing support by checking patient advice requests in provider baskets daily.
- Educate patient and document anticoagulation instructions per physician order.
- Maintain a clean, safe, and hazard free work environment; adhere to established accreditation, health system, and clinic policies and guidelines.
- Delegate tasks/supervise activities of unlicensed assistive personnel/ assist other nursing staff with patient care activities/Assist with onboarding of new clinical staff.
- Perform other related duties incidental to the work described herein.

Job Eligibility Requirements:
- Work requires graduation from an accredited BSN or Associate's Degree in Nursing or Nursing Diploma program.
- All registered nurses without a Bachelor's degree in Nursing (or higher) will be required to enroll in an appropriate BSN program within two years of their start date and to complete the program within five years of their start date.
- 2+ years of clinical experience is required.
- Adult Medical/Surgical, ED, Critical Care, Cardiac or previous Ambulatory Care experience with the last 5 years preferred.
- Must have current or compact RN licensure in the state of North Carolina. BLS required.
Job Schedule:
Monday-Friday day shift schedule with no weekends or Duke designated holidays.
