Applicable Position Description: Pharmacist Coordinator

 

Practice or research specialty/focus of the position: Anticoagulation Management

 

Position Summary:   This clinical pharmacist coordinator position in the Clinical and Patient Care Services Division provides clinical pharmacy services, including Anticoagulation stewardship, management clinical monitoring, and support of clinical pharmacy and departmental programs.  Responsibilities include:              

  • Clinical Pharmacist Practitioner (CPP) support of the Hematology Clinic 1E and support of the inpatient Hematology anticoagulation service as needed
  • Maintains outstanding skills as a clinical pharmacy practitioner and consistently demonstrates the value of pharmacy services.
  • Contributes to ongoing health-system and pharmacy department transitions of care initiatives
  • Educates medical, nursing, and pharmacy staff about the management, prescribing and monitoring of therapies used in patient care area(s) of responsibility.
  • Serves as a clinical leader to the Clinic 1E staff, Hematology Team and Department of Pharmacy for area(s) of expertise.
  • Coordinates and leads Anticoagulation Council and at least quarterly review of safety reports for specialty area/team to document opportunities for safety improvements and progress on safety initiatives within specialty team
  • Active participation/leadership in departmental and health-system committees and initiatives related to practice area as appropriate
  • Serves as liaison as needed if requested by ACPO/ Clinical Managers for Drug Shortages/Critical Shortage taskforce meetings or other specialty area related meetings or initiatives.
  • Promotes adherence to the formulary policies, procedures, state and national standards including TJC anticoagulation management standards
  • Promotes rational, cost-effective drug therapy
  • Works with peers, Clinical Managers, and ACPO to develop and achieve Division and Department goals as well as support managers with staff development within area of specialty.
  • Demonstrates the value of Pharmacy Services in their work and serves as a liaison to patients and caregivers
  • Contributes to student and resident education.
  • Provides educational lectures and/or participates in research (Duke, State/Local, National).
  • Works with peers to achieve division and departmental goals.
  • Takes responsibility for personal and professional growth.
  • Maintains familiarity with all departmental programs & works as integral part of the pharmacy team
  • Attends required meetings for Specialty Area and Pharmacy Department including but not limited to: CPCS Staff Meeting, Pharmacy Department Staff Meeting and Pharmacy Leadership Council Meeting
  • Assists with recruiting and onboarding of clinical staff in area of specialty
  • Contributes to performance evaluations of staff in clinical area of specialty
  • Additional responsibilities as assigned by Manager

 

Required or desired credentials or experience of applicants:  Pharm.D.; 5+ years of practice experience desired; PGY1 or Pharmacy Practice Residency preferred; previous experience with anticoagulation management preferred.  Licensed or eligible to practice pharmacy in North Carolina
