About Duke HomeCare & Hospice
Pursue your passion for caring with Duke HomeCare & Hospice, which offers hospice, home health, and infusion services across the region, as well as serves as the home forthe Duke Caregiver Support Program. Team members work closely with a patient's physician to provide comprehensive, individualized care in the comfort of their home or at our inpatient hospice facility in Durham, NC.

Job Responsibilities

Independently prepare complex documentation, reports, charts, graphs, and spreadsheets at designated intervals and as requested. Generates enter service requests daily. Makes corrections as necessary and informs appropriate staff if visits overlap or if corrections need to be made by other departments within the organization. Runs 3 separate Admission Statistical reports and exports to Excel. Creates IDT Agenda. Participates in weekly Case Conference

Independently prepare complex documentation, reports, charts, graphs, and spreadsheets at designate intervals and as requested. Daily reconciliation of admissions, discharges, deaths, transfers and changes in level of care.

Enter physician billing, pharmacy charges and per diems for patients claim. Make corrections as necessary and inform appropriate staff or other departments within the organization to make corrections.

Run reports and create Interdisciplinary Team Agenda. Update for the weekly meeting and notify providers of upcoming face-to-face documentation needs. Disseminate documentation to community physicians as required.

Runs Certification Expired Report for patients recertifying. Updates the recert sheet for weekly IDT meeting.

Audit Certification of Terminal Illnesses. Work closely with the Medical Director to reconcile for completeness and tracking in document manager.

Plan, coordinate and administer activities of assigned programs to include developing, implementing, and supervising related procedures, processes, services and systems; train employees in proper methods and procedures and ensure correctness of work.

Develop, plan, coordinate and implement activities including, but not limited to, seminars, conferences, workshops, short courses, clubs, shows, public events and other related programs; develop and coordinate new ideas and concepts for program themes, materials and resources to supplement, expand or replace existing program components.

Research budget variances and follow up to resolve issues.

Super User- troubleshoots staff McKesson issues and assists in resolving. Schedule Certified Nursing Assistants, Social Workers and Chaplains for all Hospice new admissions. Enters mileage, visits and medication changes for staff not documenting in McKesson. Provides admission paperwork to finance. Monitor and evaluate program effectiveness, investigate trends, and recommend and implement modifications to improve program effectiveness.

Schedule initial and on-going visits for Nurses, Nursing Assistants, Social Workers and Chaplains. Coordinate initial admission visits with coordinators and clinicians.

Enter mileage for Nurses, Social Workers, Chaplains and Bereavement staff.

Review of required forms for admission and election addendum if requested. Generate addendum of non-coverage and track for completion within timeframe.

Monitor and evaluate program effectiveness, investigate trends, and recommend and implement modifications to improve program effectiveness.

Prioritize and resolve inquiries (phone, e-mail, in person). Schedule and maintain calendar of appointments and meetings.

Act as a liaison between the department and external groups. Explain policy/procedures to other parties based on knowledge of the company and department guidelines.

Runoff bereavement letters from the monthly and make corrections to charts as needed.

Perform other related duties incidental to the work described herein.
Minimum Qualifications
Education
High school education or equivalent.
Experience
Two years of administrative support experience in increasing variety and/or complexity
Degrees, Licensures, Certifications
Proficiency in office software including Word, Excel, PowerPoint, and Outlook including the ability to design and maintain documents, correspondence, spreadsheets, graphs and charts. Must be proficient in or willing to learn the development software package, ADVANCE.
